How to Access Services
Vision Support is initiated by school administrators or by the Early Childhood Special Education team. To refer a child, submit the referral form below, and send the student’s eye report to the teacher of the visually impaired, Stacey Richart (srichart@esu1.org). They will review the eye report and determine alignment with Rule 51 verification guidelines.
If a student is eligible for services, the forms linked below will be required and a Functional Vision Assessment will be made and service recommendations will be provided.
