File Review


• Nebraska districts are on a set 5-year rotation for File Reviews.
• File Reviews are means of monitoring the implementation of IDEA (Individuals with Disabilities Education Act) and RDA (Results Driven Accountability).
• File Reviews offer the opportunity to highlight what districts are doing well and what they need to improve upon.
• Corrective action is required for Items not found in a particular file or found to be out of compliance.
• The 2014-15 school year will be the start of a new way of conducting monitoring. The steps include:

Step 1: Notification of Special Education Monitoring
Step 2: Desk Review (Contact the district to request file or files used for the desk review)
Step 3: Scheduling the Focused On-site Visit and standards to be reviewed
Step 4: Focused On-site Visit (review of additional files and interviews as necessary)
Step 5: Summary Meeting (discussion of compliance review and linkages to the ILCD Targeted Improvement Plan)
Step 6: Correction of Noncompliance
Step 7: Closeout Letter


• Nebraska Planning Region Teams (PRTs) are on a 3-year rotation for File Reviews. PRT 1 had a review during the 2013-14 school year. The next one will be conducted during the 2016-17 school year.
• The steps include:

Step 1: Notification to PRT (the PRT Lead Agency, PRT Chair, and Services Coordination Agency receives Notification/Orientation letter of upcoming monitoring schedule)
Step 2: Initial Orientation Call Meeting with the PRT (phone conversation to explain the process, share documents that will be used in the review, and share the files that were selected)
Step 3: Desk Audit (review of Rule 52 and 480 NAC 3 regulations, policies/procedures, grants, and CONNECT data reports)
Step 4: On-site Exit Conference (share monitoring results with PRT members)
Step 5: Written Notification of Findings (within 45 days of Exit Conference)
Step 6: PRT Corrective Action Plan
Step 7: Correction and Verification of Correction of Noncompliance (all noncompliance must be corrected as soon as possible)
Step 8: Closeout Letter to PRT
